The greatly revised and updated English version of
this thesis is currently (January 2011) being finalised for publication in book
form. Since the original mineographed, Dutch version was typed by hand onto
mimeo sheets, it was found to be too irregular for OCR. The Dutch internet version
therefore consists of two PDF documents containing high-resolution scans of
each page – which led to two files of such excessive length (both c. 30 Mb)
that it was not possible to continue reserve space for them on this website.
Those seeking access to these files for bona
